Abbotts Bridge Dental Care

Dental Examination
A child should have dental exam as early as possible. A baby may have one or two baby teeth when he or she is just born. It can interfere with breast feeding. It makes sense to see a dentist when the kid has baby teeth normally. The dentist may show parents how to take care of the baby teeth: for example, brushing. Besides, how to feed the baby can also have great impact whether the baby is going to have cavities.

Teeth Cleaning
Sometimes it is not true. Two days before Thanksgiving, a mom and a 2-year-old boy came to our office for exam and cleaning. Came with them is a 6-year-old sister. That latter has visited our office for several years. She gradually becomes a good little patient. While our hygienist was taking care of the mom in one treatment room, we brought the boy to another treatment room. Surprisingly, he was not afraid of the strangest person in the world. To overcome the next obstacle, we asked the elder sister to jump into the dental chair. The little brother followed it with any problem (Fig.1). Now we have the third difficulty. What is that?
Dental Sealants
In Lecture: Brushing for Kids, we discuss that the developmental grooves in the biting, buccal and lingual surfaces are susceptible areas for dental cavities (caries). To prevent the latter, we need to brush the biting surface in a 90 degree and the other two surfaces in a 45 degree. Another way to prevent cavities is to place sealant when an adult molar is just coming in the mouth of your kids. Fig.1 illustrates the biting surface of three molars. Cross lines in these 3 molars indicate the developmental grooves we just talked about. In Fig.2, the black spot represents a cavity in the middle of cross lines. Fig.3 is a cross section of an adult molar. If you are curious about how the cross section comes from, you need to return to Fig.1. The dashed red line is an imaginary line we cut through the tooth (teeth). The arrow shows how we look at the cross section. Anyway, let us look at a development groove in the cross section in Fig.3. We blow up a gray box in Fig.3 into that of Fig.4. As you may note, the groove is quite complicated, having a narrow opening and an enlarged bottom. Tiny food particles and plaque (germ film, i.e., germ product) are easily trapped there (yellow, Fig.5). The end result of accumulation of debris and germ in the groove is formation of a cavity (black, Fig.6).

Fig.1 illustrates a tooth with crown and root. The crown has two major components: cusp and fossa, analogous to mountain and valley. When upper and lower teeth bite together, the cusp of an upper teeth bites into a fossa of the opposing tooth and vice versa (Fig.2). This relationship is similar to pestle and mortar, facilitating our chewing. When our teeth do not have good alignment like Fig.3, we can grab our food as quickly as a dog does, but cannot chew very well. Our digestive system may suffer. For example, cusps are under undue stress. One day one of them is chipped (arrow in Fig.4). Or the teeth become worn too soon, i.e., cusps flattened (arrow, Fig.5). Sometimes even the tooth may be split into two pieces due to undue pressure and wear.
Bridges

Root Canal Treatment
First of all, we need to open up the infected nerve (Fig.2) and remove the latter (Fig.3). By diligent cleaning, the root canal (actually a tube inside the tooth) is basically free of debris and germs. After this, a strong alkaline paste is placed inside the tooth (yellow, Fig.5) and sealed with cotton (pink) and temporary filling (gray). The alkaline paste can further kill remaining germs and clear the root tip infection (light blue, Fig.6). The paste also promote root tip closure (dark blue, Fig.7). The doctor may need to change the dressing every one or two months until the root tip is close.

Dentures
After extraction a small piece of bone may protrude underneath the gums adjacent to the extraction area, produce severe pain and interfere with denture fitting.
