Overall

The office space is clean. The reception and nurses are polite and professional. Dr. Walker is awesome! She listens to all of your concerns and explains things to you so you can understand. She is very personable and professional.

Dr. Costley and staff are great. It's a small practice, so they really take the time to know you and your medical history. It's very convenient, and easy to get an appointment!

Brooke is a family nurse practitioner who joined Ponce Primary Care in 2020. She completed her Bachelor’s and Master’s of science in nursing at the University of North Georgia. Brooke’s medical interests include pediatrics, women’s health, chronic disease management, and preventative medicine. Having been a registered nurse for many years on a cardiac intensive care unit she also has a strong passion for managing cardiovascular disease. She enjoys learning, and strives to stay up to date with advances in medicine to ensure her patients maintain a healthy lifestyle. Brooke lives in Atlanta with her husband and enjoys outdoor activities, reading, and weekends in the North Georgia mountains.

Family Practice
Dr. Costley is the founder and co-owner of Ponce Primary Care and remains active in a clinical leadership role with the practice. He earned his MD degree at the University of Michigan where he also completed his post-graduate residency training. He is board-certified in Family Medicine and has 20+ years of clinical experience in the breadth of primary care medicine. Dr. Costley also holds a MBA from Emory University and serves as the CEO of BlueWillow Biologics, a small biotechnology company developing intranasal vaccines. While Dr. Costley is not currently accepting new patients, he is engaged daily with Ponce Primary Care's provider team in the care of our patients. He is married with two adult children and enjoys fly fishing, reading and working to improve his Spanish.
Emergency Care
We attempt to maintain daily calendar openings for unanticipated sick care needs, such as acute illnesses that require timely attention. However, if you are experiencing any life threatening symptoms, or if it is outside of our operating hours and you are in need of immediate medical attention, you should visit the appropriate emergency room.
Immunizations
We ask most patients to see us once a year for a visit focused on prevention issues. For some younger patients in good health, these visits can occur every couple of years. Insurance companies generally allow one such visit per year which is not subject to copays and deductibles (there are insurance exceptions). It is usually not possible to cover the full range of indicated screening tests, lifestyle recommendations, immunizations etc. while also addressing medical problems during a single visit.

Gynecology
We provide both preventive and problem-oriented care for women. Services include routine gynecology care (pelvic and breast examinations, pap smear testing), contraception care (including IUD and Nexplanon placement), sexually transmitted disease screening and treatment, treatment of menopausal symptoms and management of a variety of gynecologic problems. We have excellent relationships with a number of gynecologists when referral for specialty care is needed.

Internal Medicine
The past 5 years she has been working in internal medicine and enjoys working with patients to optimize and prevent illness with a combination of lifestyle modifications and medications when necessary. She is passionate about chronic disease management including diabetes, obesity, and cardiovascular disease. She recognizes that everyone's health and circumstances are unique and regularly takes a shared decision making approach with regards to treatment decisions.

Attention Deficit Hyperactivity Disorder (ADHD)
Stimulants for ADD/ADHD treatment: We will continue to prescribe appropriately dosed stimulants for the treatment of diagnosed ADD/ADHD. Patients receiving these medications will need to have an appointment with one of our providers every 3 months. We now offer virtual (i.e. telemedicine) visits which are can be appropriate for these visits. However, one in-office visit each year is required in order to assess for unintended effects of chronic stimulant use (e.g. hypertension). Patients who have not previously been diagnosed with ADD/ADHD will require a specialist consultation to establish the diagnosis prior to starting a stimulant medication.
