Ear Infections
Deep in the ear canal is the eardrum or tympanic membrane. This is a minimally transparent membrane that vibrates with sound, which moves tiny bones, the smallest in the body, attached to nerves, which transmit signals to the brain that gets interpreted as words, music or whatever. When we speak of â€œear infectionsâ€ and usually mean infections of the middle ear. This is called otitis media. Otitis media is very common in children and gets less common as we get older as the Eustachian tube that drains the middle ear gets bigger and less likely to swell shut.
Sleep Disorders
The recognition that babies have a much lower, almost nil chance of Sudden Infant Death or Crib Death when they sleep on their side or back was brilliant.Â It seems like the risk was of breathing into blankets and pillows and rebreathing carbon dioxide.Â Amazing.Â I am not opposed to bringing the baby into your bed at night.Â A sober parent with no sleep disorder will not suffocate a baby by rolling on top of it.Â Do what seems prudent and practical for your family.Â The adults need to maintain their relationship with each other and need to come to agreement about this arrangement.Â If everybody sleeps better in a â€œfamily bedâ€ , then that might be right for your family.Â But only if it makes everybody happy.Â The adults will be stuck with each other for years after the kids are grown and gone.

Colonoscopy
If constipation is a chronic problem for you, seek a medical opinion. Sometimes conditions like low thyroid or poor nutrition can affect intestinal motility. If your constipation is a new problem for you or to a degree you have never before experienced, seeing a doctor is a really good idea. You may need further testing. Blood work, x-rays and even a colonoscopy may be in your future.

Babies should be seen frequently because they change so much so fast.Â You really need an objective trained observer to periodically assess your child, even if you are going to decline vaccination.

Dave Barry wrote a very funny book called Babies and Other Hazards of Sex.Â It is full of humorous insights into pregnancy and childbirth.Â But after childbirth is when the real problems start.Â The darn things do not come with an instruction manual.Â The hardest thing is trusting yourself.Â No one knows your baby like you do.Â There is no one right way to do things, so as long as youâ€™re thoughtful and not doing anything dangerous, youâ€™re doing whatâ€™s right for your family.Â Trust that.
Labor and Delivery

Youâ€™ve seen it.Â The recommended optimal blood pressures keep getting lower, as does the upper limits of recommended blood sugar and cholesterol.Â It used to be that pregnant women were given cigarettes and diet pills to control weight gain.Â The recommendations for which medicines to use for hypertension is in constant flux.Â We have shifted the diagnostic criteria for illnesses like bipolar disease and autism making an incredible jump in the incidence of these disorders.Â And donâ€™t get me started on hormones....

Stress Management
Not getting behind (so to speak) in this area is always preferable to trying to catch up after getting into trouble. Adequate hydration, daily exercise, and a healthy diet all can contribute to good intestinal health. And as always, stress doesnâ€™t help anything, so stress management and relaxation are vital. (Some people get loose bowels with stress and/or anxiety and some people bind right up.)

My firm belief is that Western Medicine, although not great at many things; allergies, chronic pain, mental health and nutrition, to name a few, has done some things very well. Immunizing against deadly diseases is one of them. Vaccination as preventative care is a huge benefit to global health, not just here in the US. It is so amazing to me that we have the ability to give our bodies a warning about a specific threat, so our immune systems can create a specific weapon against that threat. I just love preventing disease. Children have particularly strong immune systems. They have to. They are constantly on the floor, eating dirt and anything else they find to put in their mouths. They get cuts and scrapes all the time. Their immune systems are much more vigorous than grown ups. That makes childhood a perfect time to tap into that and use its strength to create lifelong protection.

Physical Therapy
There are some driving assessments for seniors. Sometimes this can be helpful in being objective when there is uncertainty. The ones I know about are through larger physical therapy/rehabilitation centers and some states licensure centers. It costs money and is not a medical insurance covered service. A pretty firm guideline is if you have been lost while driving in a familiar area, even once, you should not be driving AT ALL. Let the doctor be the bad guy and determine that an elder canâ€™t drive. When the discontent arises, blame the doctor. You donâ€™t have to agree or disagree. You donâ€™t have to take sides. You donâ€™t have to directly take the heat.
