The Form House manufactures products in the binary industry. It is a division of Diam International. The Form House s products and services include publication inserts, plastic card encoding and personalization, plastic card affixing, product sample affixing, label affixing and project management. The company has 140 manufacturing lines and has the capability to custom finish, inkjet address and shrink-wrap. The Form House s 320,000- square-foot facility is centrally located in Chicago. The Form House has more than 100 conveyer affixing machines, 40-plus labeling machines and nearly 10 3M POST-IT note machines. It produces more than 1.2 billion pieces per year for many national advertising agencies such as Leo Burnett USA, McCann Erickson, Chiat Day, Foote Cone & Belding and Darcy Masius Benton & Bowles. The Form House was founded in 1952 specializing in finishing one-time carbon forms. It s been a vendor on projects for Procter & Gamble, McDonalds, IBM, Disney, Hallmark, DaimlerChrysler, Pillsbury, Target, Wal-Mart and Helene Curtis.
