General Info: Founded in 1977, the Chicago Family Health Center is a community-based, nonprofit health center that provides primary health care services to the residents of Chicago. It operates a health resource center that offers health information and screening services to students and faculty. The center provides medical, dental and mental health care services to more than 17,000 patients. It also offers outreach, patient education and case management services. The center treats patients with developmental disabilities and chronic diseases, such as diabetes and asthma. Additionally, its maternal and child health team includes physicians, nurses, case managers, outreach workers and breast-feeding counselors. Chicago Family Health Center provides medication to uninsured patients through several pharmaceutical companies. It is located in Chicago.
