For over thirty years, Project Transition has provided hope and healing to adults with serious mental illness. Our members are part of a residential treatment program that promotes sustainable recovery from mental illness. Each mental health treatment program is located in an apartment setting with several locations throughout the Greater Philadelphia Region. All of our programs are fully licensed and are also fully accredited by The Joint Commission. Many of the people we work with have struggled with previous treatment efforts, and have experienced extensive stays at psychiatric hospitals and drug and alcohol programs. We provide a supportive therapeutic environment that enables our members to transition in functioning members of the community.

We have six programs located throughout Philadelphia, Bucks, Montgomery, and Berks Counties. Our Administrative Office is located in Chalfont, PA.
