- Caring Partners Home Health is a subsidiary of the nationally recognized Glacier Hills Senior Living Community. At Caring Partners Home Health, we provide unmatched skilled health care and private duty services based on our extensive knowledge, experience, and innovative practices. We’re by your side with the help you need and the care you deserve. A nonprofit organization service Southeast Michigan since 2004. Your Home Health Team: We provide both Private Duty and Skilled Services. Skilled Nurses- provide assessments, nursing care, medication administration, wound care and disease management. Physical Therapists- will teach you home exercises to regain strength and prevent falls. Occupational Therapists- assist with home safety and help you learn how to bathe independently and work in your home. Speech Therapists- provide treatment for speech and swallowing disorders. Medical Social Workers- will assist with counseling, long term planning and access to community resources. Home Health Aides- help you with bathing, grooming, and personal care. Our nurses and therapists are licensed in the State of Michigan, and all of our staff members participate in ongoing training and skills validation on a regular basis. Special Programs Telehealth Dementia Care Your Personal Care Team: Personal Care- trained caregivers assist with bathing, dressing, grooming and activities of daily life. Home Management- help with light housekeeping, meal preparation, shopping, and transportation to medical appointments. Companionship- our companions will be there for daily support and interaction, and accompaniment to social functions and activities. Flexible Staffing- provides coverage tailored to you needs, from 3 to 24 hours per day. Detailed Nurse Assessments- our caring nurses determine your unique needs and create a plan of care that’s right for you. Nursing Oversight- regular review of your care plan and ongoing supervision of caregivers ensures your satisfaction with our services. Care Management- private duty nurses will oversee medication management and coordinate your health care and well being.
