General Info: A day in Lehigh Valley Health Network’s Adolescent Transitions Partial Program consists of morning and afternoon groups, focusing on therapy and skill-building workshops. Our goal is to empower adolescents and their caregiving system through tools that provide ongoing support after discharge. Our collaborative multidisciplinary approach allows patients to leave the treatment experience with evidence-based strategies and skills to continue their progress. Medication assessment and management also are integral parts of treatment. Let us be your safe haven, where we can partner together without shame, guilt or judgment, and you can get back to living your life. We understand and can help with: -Anxiety -Depression -Mood instability -Emotional regulation -Suicidal ideation and self-injurious behaviors -Relationship or family concerns -Educational stressors associated with bullying, social media and peer pressure -Medication management Our partial program is person-centered -Patients receive focused attention every day in an environment of support, reducing the need for more intensive levels of care. -Treatment is delivered by a multidisciplinary treatment team collaborating with the patient and their family. -Our holistic treatment approach encompasses an individualized length of stay and allows patients to return to the comfort of their home every evening. -Patients learn practical tools to help them confidently cope with day-to-day challenges after discharge.
