Emergency Care
In rare cases, an erection may last too long. This can badly damage the blood vessels in your penis. If an erection lasts longer than 4 hours, go to the emergency room right away.

Urinary Incontinence
To help treat stress urinary incontinence (SUI), your surgeon may perform a procedure called pubovaginal sling surgery. A “sling” of tissue is placed like a hammock under the bladder and urethra. The sling helps support the bladder and urethra firmly in place. This helps keep the urethra closed to prevent urine leakage. Your surgery will take about 2 hour(s). You will be asked to do some things at home to prepare for surgery. Below are guidelines to help you get ready. If you have any questions, call your nurse or doctor.

The pelvic floor muscles may weaken due to aging, pregnancy and vaginal childbirth, injury, surgery, chronic cough, or lack of exercise. If the pelvic floor is weak, your bladder and other pelvic organs may sag out of place. The urethra may also open too easily and allow urine to leak out. Kegel exercises can help you strengthen your pelvic floor muscles so they can better support the pelvic organs and control urine flow.

Bladder Cancer
As cancer cells multiply, the tumor grows. Bladder cancer begins in the lining of the bladder, and often doesn’t grow beyond that layer. As the tumor gets larger, it may invade (grow into) deeper layers of the bladder. It may also invade nearby organs, such as the prostate in men or the uterus in women. Cells can break off from the main tumor and enter the bloodstream or lymph nodes. Blood or lymph then carries the cells to other areas of the body, such as the bones, liver, or lungs, where a new growth may form. This process is called metastasis. The stage of cancer is based on where the cancer is and how much it has grown and spread. The stage is found by looking inside the bladder during cystoscopy and using tests that show images of the bladder, the areas around it, and parts of the body that the cancer may spread to. The staging system described below is a simplified one. Your doctor will most likely use a more detailed system.
Urology
Dr. Pfeffer joined Fauquier Health as a private practice physician with Urological Associates of the Piedmont in 1991. Prior to that, he was an assistant instructor in the Urology Department at Brown University, and a clinical associate instructor in the Surgery Department at Cornell University.
Nephrology
Kidney Stones
If you’ve had a kidney stone, you may worry that you’ll have another. Removing or passing your stone doesn’t prevent future stones. With your doctor’s help, though, you can reduce your risk of forming new stones. Follow up with your doctor to help detect new stones. You may need follow-up every 3 months to a year for a lifetime.
Urinary Tract Infection
A urinary tract infection (UTI) is due to germs called bacteria growing in the urinary tract. It can cause blood in the urine. Other symptoms include burning or pain with urination. You may need to urinate often. You may also have a fever.
Prostate Cancer
Problems with the prostate (a gland in the male reproductive system) become more common as a man ages. These problems include prostate cancer, a common cancer in men. This cancer can often be cured or controlled, especially if it is found and treated early. Screening tests help detect prostate cancer before it causes any symptoms.

Ultrasound
Ultrasound is an imaging test. It uses sound waves to form pictures of your organs that appear on a screen. Transrectal ultrasound uses a special probe that is placed directly into the rectum. This test lets your doctor assess the prostate gland, which is in front of your rectum. During the test, tissue samples (biopsy) may also be taken.
MRI
If cancer is found, imaging studies may be done to check for its spread. Bone scans(x-rays) check whether cancer has spread to bones. CT and MRI can detect tumors in bones and soft tissues.

Intravesical therapy is often done in a doctor’s office. A flexible tube (catheter) is used. It fills the bladder with a liquid medication. This may be a liquid chemotherapy drug. It kills cancer cells. Or it may be BCG. This is a type of bacteria that helps boost

After earning his Doctor of Medicine degree from the Washington University School of Medicine in 1983, Dr. Pfeffer completed the North Shore University Hospital General Surgery Internship and Residency Program, and the Rhode Island Hospital/Brown University Urology Residency Program. Prior to medical school, Dr. Pfeffer earned a bachelor’s degree in engineering from Johns Hopkins University.
Vasectomy
A vasectomy is an outpatient procedure. This means you’ll go home the same day. It’s done in a doctor’s office, clinic, or hospital. Before your procedure, you’ll be asked to read and sign a consent form. This form states you’re aware of the possible risks and complications. It also says that you understand that the procedure, though most often successful, can’t promise to make you sterile. Be sure you have all your questions answered before you sign this form. Below is a list of risks and possible complications of the procedure.
