Urinary Incontinence
The USPTF has overstated the risks associated with prostate cancer diagnosis and treatment. Serious, life-threatening complications occur in less than 1 in 1000 prostate biopsies. Urinary incontinence occurs in less than 5-percent of cases when treatment is administered by experienced specialists. Erectile dysfunction following treatment for prostate cancer is serious, but not life-threatening. Additionally, erectile dysfunction can be treated in almost every case when it does occur.

Bladder Cancer
Cystoscopy is a simple and common procedure that involves looking into the bladder using a small, fiberoptic scope inserted through the urethra. The procedure takes less than 2 minutes. The scope has a camera attached that allows the patient to see everything the doctor is seeing during the procedure. Cystoscopy is commonly used to evaluate blood in the urine, bladder cancer, recurrent infections and prostate obstruction.

Kidney Cancer
Kidney cancer presents in many shapes and forms. However, the presentation is rarely directly associated with the cancer itself. For example, a patient may happen to find an abnormal growth on the kidney during an CT evaluation for gall stones. Most kidney cancers must grow to a considerable size before they cause pain or can be felt on physical examination.
Kidney Stones
Americans have a one-in-10 chance of developing a kidney stone in their lifetime. This makes kidney stones one of the most common medical problems someone can have. Most kidney stones are composed chiefly of calcium. Dehydration is the most common cause of kidney stones.
Testicular Cancer
Testicular cancer is a fairly rare, but potentially deadly form of cancer that typically occurs in young, Caucasian males. It most commonly develops as a painless growth or enlargement of the testicle. Progression and spread of testicular cancer can occur rapidly, compared to many other forms of cancer, so rapid diagnosis and treatment are critical.
Interstitial Cystitis
Interstitial Cystitis (IC) is a bladder condition characterized by frequent, painful urination, as well as a small bladder capacity, pain with intercourse and a feeling of constantly needing to go to the bathroom. It is not caused by infection, so typically, patients with IC do not respond to antibiotic treatment.
Urinary Tract Infection
Symptoms of urinary tract infection (UTI) may include frequent urination, burning on urination (dysuria) or urinary urgency. Patients may urinate often but only pass a small amount of urine and may not feel they empty completely.
Prostate Cancer
The benefit of PSA screening is starkly illustrated by comparing the prostate cancer death rates between countries where PSA testing is and is not performed. Sweden, for example, does not perform PSA testing and has the third highest prostate cancer death rate in the world. The U.S. ranks forty-sixth in the world (and falling!), in large part due to our widespread practice of PSA screening. Incidentally, the U.S. ranks first in the world for prostate cancer survival.

Multiple Sclerosis
Urge incontinence is now commonly referred to as "overactive bladder" and is caused by the bladder muscle contracting and expelling urine when it shouldn't. Many medical conditions can result in urge incontinence including stroke, Parkinson's disease, multiple sclerosis and prostate enlargement. However, "overactive bladder" can also occur without any of these more serious problems. Urge incontinence is easily treated with any of a variety of medications. The newer oral ones include Ditropan XL, Detrol LA, Vesicare, Enablex, and Sanctura. Oxytrol is a patch worn on the skin for overactive bladder.

Ultrasound
The American Urologic Association has recognized Dr. Steve Johnson's mastery of urologic ultrasound with a Level II Verification of Skills certificate. He was the first urologist in Grayson County to attain such verification. Dr. Johnson attended a hands-on ultrasound course sponsored by the AUA to obtain the verification. The 2-day course featured 12 hours of CME lectures on the basic and advanced aspects of urologic ultrasound imaging. The certifying portion of the course included written tests and hands-on testing demonstrating mastery of urologic ultrasound imaging techniques.
Computed Tomography
Symptoms include pain in the flank (below the ribcage on the right or left side), nausea and blood in the urine. In severe cases, fever and infection may develop which can become life threatening. Diagnosis is usually confirmed with a CT scan although the more "old-fashioned" IVP may be obtained on occasion as well.
Chemotherapy
Early, less aggressive bladder cancers are limited to the bladder lining and are considered "superficial". Superficial bladder cancers are typically cured with the procedure described above. However, since 40 to 60 percent of bladder cancers recur, further office treatment may be recommended in certain cases. This treatment is an immune therapy called BCG. BCG is not chemotherapy. BCG is medication placed into the bladder using a small catheter once a week for 3 to 6 weeks. The immune system is activated by the BCG which lowers the risk of recurrent cancer 50 percent.
Radiation Therapy
Early prostate cancer is curable in a high percentage of cases (75-85%). There are many treatment options for early prostate cancer including surgical removal, cryoablation (freeze therapy) and radiation therapy.

Vasectomy
Vasectomy is often requested by men who do not wish to have any more children in the future. A small incision is made in the scrotal skin and the vas deferens (which carry sperm from the testicles to the urethra) are cut on each side. The procedure can be performed easily, quickly and comfortably using a local anesthetic. Men should consider vasectomy a permanent method of birth control since reversal is very expensive and is not guaranteed to restore fertility.
Hip Replacement
One lesser known type of incontinence is overflow incontinence. Overflow incontinence is caused by a bladder that is full to capacity and cannot empty. Any further urine production from the kidneys "overflows" out of the body. Overflow incontinence can lead to infection or kidney failure. Patients with overflow incontinence will often have a long history of diabetes, severe prostate obstruction or recent major surgery (hip replacement, hernia repair, etc.). Patients with overflow incontinence should have a drainage catheter placed and be seen by a urologist in short order.
Laparoscopy
While surgery for kidney cancer has traditionally involved an incision with or without removal of a rib, many new techniques are now commonly used in select cases. These techniques include laparoscopy, in which the surgery is performed using a scope and small instruments inserted though small incisions in the skin, as well as cryoablation where the tumor is destroyed by freezing and radio frequency ablation where the tumor is destroyed using high frequency radio energy.
