In the most basic terms, bunions form when your joints and tendons bear weight unevenly, putting extra pressure on your big toe joint and causing it to fall out of alignment. The root cause could be one (or more) of many things—an inherited structural defect in your foot, flat feet, ill-fitting shoes, rheumatoid arthritis—but the result is the same: a big toe pushing towards the smaller toes and a hard, ugly knob on the side of your foot. The condition develops slowly, but eventually it can begin to cause pain and swelling, limit your mobility, and can cause secondary conditions such as bursitis, corns, hammertoe, and metatarsalgia.
Plantar Fasciitis
Within a few minutes, we are able to evaluate and diagnose a multitude of problems such as neuromas, tendonitis, bursitis, bone spurs, ulcers, arthritis and plantar fasciitis and how extensive the pathology may be. The images are captured to our patients’ charts and we then have an opportunity to discuss and implement a comprehensive treatment plan immediately.

Calluses
Diabetics often experience very dry skin on their feet, sometimes leading to peeling, deep cracks in the skin, or calluses. Nerve damage is often the culprit for these skin condition changes. Bunions are a relatively common foot problem, with as many as 1 in 30 adults under 30 and up to 1 in 6 adults over 60 suffering from some form of the condition. Over time, uneven pressure on the foot causes the big toe to drift toward the second toe, enlarging the joint at the base of the big toe and pushing it outward, reshaping the inside of your foot.

Flat Feet
However, you could develop this condition from a painful injury or another problem that affects the tissues supporting the midfoot. These “fallen arches” are called adult acquired flatfoot deformity. They can appear in just one foot or in both feet at once. Usually, the problem is the result of posterior tibial tendon dysfunction, injury to the bones or ligaments in the arch, arthritis in the tarsals, or diabetic foot collapse. As your feet fall inward, the discomfort becomes more pronounced and may make it difficult to participate in high-impact activities.

Ingrown Toenails
If you catch an ingrown toenail early, you might be able to easily treat it at home by soaking your foot in warm, soapy water several times a day until the condition improves. Avoid wearing tight shoes, and monitor your toe for signs of improvement. If it gets worse, or you see signs of infection, such as redness and swelling, or severe pain, it’s best to seek treatment from a qualified podiatrist like Dr. Lukoff.

Metatarsalgia
The ball of your foot is also called the metatarsal region. When you look down at the top of your foot, you may see or feel five long bones coming down to the center of your foot. These are the metatarsal bones that connect your toes to the rest of your foot. They help to distribute your weight evenly, keep you balanced, and provide acceleration when you move. The impact on this area can be intense and without sufficient rest, or with a faulty foot structure, the metatarsal area can become very overworked and cause pain. When this happens, we refer to ball of foot pain as “metatarsalgia.”

Foot orthotics are custom foot supports. They are designed to fit your feet more efficiently than over-the-counter (OTC) arch supports and balance the biomechanical inadequacies of your feet and legs. Custom orthotics are designed specifically for your feet, gait and biomechanics. OTC arch supports come in a standard size. While they will match some peoples’ arches, others may find the support too high or low, or too far back or forward. Our custom orthotics vary depending on the type of sports, activities and athletics are patients engage in. They also vary depending on the type of shoe, sandal or boot our patients wear. We use different labs for advanced technologies such as our Dynaflange orthotic that literally puts a “spring” in your step. Whatever your needs may be, we have the perfect orthotic for you.
Plantar Fasciitis
Plantar fasciitis is most common for people who have structural problems such as flat feet or high arches. Additionally, wearing unsupportive shoes or spending many hours a day on a hard, flat surface can put strain on the plantar fascia and lead to heel pain and discomfort.
Podiatric Deformities
Foot deformities such as bunions and hammertoes can add extra stress on the metatarsals, as can poorly fitting shoes. High heels are common culprits, as they force all of the body weight onto the ball of the foot. Wearing shoes with narrow toe boxes or even athletic shoes without the proper support or cushioning can lead to metatarsalgia. Finally, being overweight, having a stress fracture in your foot, and Morton’s neuroma are also contributing factors.

He completed his residency in podiatric medicine and foot surgery at Central Hospital in Somerville, Massachusetts.
