        No care plan meetings during t...

        No care plan meetings during the entire 4 month stay (which is a requirement) in rehab despite requests to be involved in my mother's care.

        Discharged with UTI (untreated). Calls to therapists were not returned. Nursing staff always acted too busy to answer questions. CNA's always saying "we are understaffed today".
