My mother was recently admitted to this home; they have lost all the items she brought with her from the hospital and claim never to have received them. Mom saw them in the transport van. These are essential items: a leg brace and special shoes. The call buttons are NOT answered promptly, if at all. She has requested a transfer to another facility in the area and has been labeled uncooperative because of it. The facility that she requested transfer has received no

paper work from this Scott Street facility. Her meds are not administered promptly and they claim to have no doctor orders for her rehab and for treatment of the infected area on her foot. My calls into her are treated poorly, telling me

to call other numbers which are then not answered. I had to call four (4) times last night to speak with her. I was told the portable phone was being charged--do they have only one phone for speaking to residents? Please be fore warned:
