Urinary Incontinence

* Stress incontinence. This leakage of urine with stress or an increase in abdominal pressure when you exert pressure on your bladder by coughing, sneezing, laughing, exercising or lifting something heavy. In women, causes of this type of incontinence include physical changes resulting from pregnancy, childbirth and menopause. In men, it may be caused by removal of the prostate gland.

Bladder Cancer
It is an insidious cancer that at the initial stage could be asymptomatic and the only early sign could be a one time episode of urine mixed with blood (gross hematuria) that needs immediate attention of an Urologist. Treatment with PO antibiotics could only delay treatment and deprives the patient from a chance to be diagnosed and treated early. Another episode may follow months later and the cancer may be at an advanced stage, beyond cure.
Urology
General Urology is a surgical specialty that focuses on the urinary tracts and genitourinary organs of males and females. It focuses on the diagnosis and treatment of diseases related these organs including the kidneys, adrenals, ureters, bladder, penis, testes, and prostate.
Kidney Cancer
The kidney cancer can be diagnosed with great accuracy by performing a contrast CT scan. Most of the tumors do not require a biopsy but on occasions with small solid lesions less than 3 cm in diameter a biopsy is necessary. Many kidney cancers are diagnosed while investigating the causes of microscopic or gross hematuria.
Kidney Stones
Kidney stones are a quite common urologic disorder. It is a hard mass that developed in the collecting system of the kidney. It usually starts in the microscopic tubules collecting urine from the renal pyramid and forms a nucleus of crystallization that attracts more and more crystals of either uric acid or oxalates. In time a small nucleus increases in size. It is called the Randall’s plaque that with time becomes bigger, it may detach and grow in size to form an urinary calculus. Calculi less than 3 mm in diameter may pass spontaneously through the urinary system causing little discomfort.

Pelvic floor spasms occur when the muscles that surround the urethra tighten. Treatment includes warm baths, exercises, and, occasionally, medication and/or physical therapy. IC (interstitial cystytis) is a condition in which the bladder becomes inflamed and irritated. Symptoms include frequency of urination and pelvic pain, with some women’s symptoms often getting worse during menstruation. There are many successful interventions for treating Interstitial Cystitis, so consultation with a urology specialist is recommended.
Urinary Tract Infection
Painful urination, lower abdominal pain, foul smelling urine, burning during urination with or without increased temperature are usually the symptoms of lower urinary tract infection.
Prostate Cancer
Symptomatic cancers signify more advanced stage and poorer outcomes. That is why the advent of performing a PSA in patients who are at risk permitted early diagnosing at early stages of Prostate Cancer. Patients with a family history of prostate cancer amongst blood relatives (sons of fathers with CaP, paternal Uncles with CaP brothers of patients with CaP) are at higher risk to develop CaP as well as Afro-Americans.

* Urge incontinence. This is a sudden, intense urge to urinate, followed by an involuntary loss of urine. With urge incontinence, you may feel the need to empty your bladder frequently, especially at night. This may be caused by urinary tract infections, bladder irritants, bowel problems, Parkinson’s disease, Alzheimer’s disease, stroke, injury or nervous system damage associated with multiple sclerosis.

It is an insidious cancer that is asymptomatic even in advanced stages and most of the times is discovered by chance while a sonogram or a CT scan was performed for totally unrelated reasons. The widespread use of abdominal ultrasounds and CT scans of the chest, abdomen and pelvis performed to evaluate a wide range of complaints permitted to discover many unsuspected kidney cancers at their various stages.
Chemotherapy
For patients who were unfortunate to develop metastatic disease Dr. Plawner is offering medical treatment consisting of giving injections every 3-4 months of Lupron in combination with Casodex to contain the cancer. At the same time Dr. Plawner manages the patient’s bone health because of osteoporosis that follows this type of treatment. For those unfortunate patient’s whose cancer keep progressing Dr. Plawner works with Oncologists who treat the cancer with chemotherapy.

Minimally Invasive Surgery
Patients who had multiple abdominal surgeries who developed dense intraperitoneal adhesions placement of Robotic Ports is dangerous. It may lead to damage of intestines, internal organs or major vessels (aorta, vena cava). These patients may undergo another minimally invasive surgery via perineal approach. The results, short recovery time and minimal blood loss are similar to Robotic Surgery.

Robotic Surgery
While performing minimally invasive robotic surgery the surgeon sits at a console next to the patient and controls four robotic arms. One arm holds a three-dimensional camera and controls its movements, while the other three hold miniaturized surgical tools, which precisely replicate the surgeon’s hand and finger movements.
Laparoscopy
Robotic surgery is the most technologically advanced and minimally invasive approach to complex surgical procedures available today. Before the surgical robots became available, Urologists performed operations on kidneys, bladder and prostates through large abdominal skin incisions. This meant a larger blood loss, longer hospitalization and prolonged recovery. With the advent of laparoscopy that all changed but laparoscopic surgery required a long learning curve and young physicians were required to take 2-3 additional years of training to become proficient.
Reconstructive Surgery
Depending from the underlying condition the patients are offered medical treatment with oral medications (OAB), some candidates are selected for Pelvic Floor Rehabilitation Program and some with severe prolapse of the pelvic organs are offered reconstructive surgery.

Lesions
It usually consists of performing a CT scan with contrast that permits to outline all details of the urinary tract and of cystoscopy (looking into the bladder with a fiberoptic instrument with a micro camera).
