At Hospice Compassus, care is provided by team of health care professionals who are skilled at serving people with life-limiting illnesses. Our family-centered approach focuses on the patient, led by an interdisciplinary team, including a medical director, nurses, social workers, non-denominational chaplains, counselors and trained volunteers. Aimed to enhance the quality of life remaining, our care team works to provide expertise in pain and symptom management and to meet the medical, emotional and spiritual needs of patients and their families. Our goal is to provide the greatest possible comfort and care for those who experience one of life's most intimate and challenging moments. At Hospice Compassus, we honor that every life is unique and deserves a meaningful and personal final journey; and, it is our mission to serve our patients and their families with compassion, integrity and excellence. - * On-call service available 24 hours a day/7 days a week * Fully covered by Medicare/Medicaid and most private insurance * Consultations and Admissions 7 days a week * Physician directed comfort care * Specialized nursing care and RN case management * Personal physician, hospice Medical Director and RN coordinated care * Experienced care team in pain and symptom management * Medical social services * Spiritual counseling available through non-denominational Chaplains * Bereavement counseling and grief support services and groups * Certified Hospice Aides to provide personal care * Day-to-day medical care and symptom management * Dietary consultation * Light housekeeping and meal preparation assistance * Local call nurses * Non-medical volunteer support and programs * Support Therapy Services for comfort * Regularly-scheduled nursing visits * Respite care is available through an on-site inpatient unit (select AZ locations) * Advanced Directive and Health Care Power of Attorney guidance * Community and professional continuing education programs offered regarding end-of-life care, pain management, and symptom control. * End-of-life care provided wherever you call home, whether it be a hospital, nursing home, assisted living center, personal care home or your personal residence * Assistance with communication challenges due to hearing, speech, visual impairment or inability to communicate in English available.
