Outreach Health Services is a Texas-based health care company that provides a wide range of services to people of all ages. Since 1975, the company has been providing dignified, affordable and high-quality home health care to the elderly, disabled and their families. Since 1993, Outreach has dedicated itself to giving Texas children a healthy head start in life through a broad range of services in it’s Women, infants and Children (WIC) program. The company continues to be sensitive and sympathetic to the needs of Texans in every county throughout the state. To meet the growing needs of our clients, our team of caring professionals offers services thorough several program areas: The Community Care Division provides a wide range of services to our clients from personal grooming to housekeeping to home modifications - all in the comfort and security of their homes. The WIC Division provides nutritional support and guidance to eligible Women, Infants and Children (WIC), along with breastfeeding instruction, high risk dietary counseling, food and infant formula supplements and other valuable services through its more than 70 WIC Clinics statewide. The Consumer Directed Services (CDS) Division provides Financial Management Services to Community Care Clients who choose to manage their own care. Outreach processes payroll, makes payment for approved purchases, prepares a client budget, and monitors the sue of budgeted funds throughout the Individualized Service Plan. The Pediatric Care Division provides home health care services and support, including nursing and attendant care, for medically fragile children, and respite care for families. The Emergency Response Services (ERS) Program offers a 24-hour monitoring and communication system, allowing the homebound elderly and disabled to live independently in their own homes, safe and secure.
