We will provide quality home health care services to patients who are capable of independent living but need support services. OUR PLAN We focus on treating the patient for their health promotion and maintenance. We serve patients so that they feel ensured and strengthened to continue their daily activities. With our full time staff we hope to: To perform the initial assessment of each new patient to determine their individual needs To develop a care plan for the patient To determine and designate the type of aide services the patient requires.
