Internal Medicine
Dr. Swinehart, a chemistry major at the University of California Berkeley from 1965 to 1969, received his M.D. from the Creighton University School of Medicine in Omaha, Nebraska in 1973. He served an internal medicine internship at Creighton University from 1973 through 1974, and then was an emergency room physician in Reno, Nevada, from 1974 through 1975. Dr. Swinehart took his dermatology residency at the University of Colorado Health Sciences Center in Denver between 1975 and 1978, serving as chief resident in 1978. He has been in private practice in Denver, Colorado since 1979 and has treated skin cancer during this entire time. Dr. Swinehart has specialized in Mohs surgery since 2006 and is a Fellow of the American Society for Mohs Surgery. Dr. Swinehart has also performed numerous outreach clinics in eastern and southeast Colorado for the treatment of skin cancer. He is a Fellow of the American Academy of Dermatology, a member of the American Society for Dermatologic Surgery, a member of the International Society for Hair Restoration Surgery, and is a past president of the Colorado Dermatologic Society.

Every 2 or 3 weeks in my practice, I hear a variation of this story: "Dr. Swinehart, my aunt had a changing mole two years ago and showed it to someone else, who said that it was 'nothing.' A year later, she saw a dermatologist who diagnosed melanoma and removed it. However, it has now spread to her lungs and liver, and she is in the hospital receiving chemotherapy."

Mohs Surgery
It is important to note that Mohs surgery is not appropriate for the treatment of all skin cancers. Mohs micrographic surgery typically is reserved for those skin cancers that have recurred following previous treatment or for cancers that are at high risk for recurrence. Mohs surgery also is indicated for cancers located in areas such as the nose, ears, eyelids, lips, hairline, hands, feet, and genitals, in which maximal preservation of healthy tissue is critical for cosmetic or functional purposes.
Reconstructive Surgery
However, the early operative techniques were certainly not without problems. The old "dry technique", performed under general anesthesia without any local anesthesia, seemed to often produce "nothing but blood", limiting the amount of fat removal. The "wet technique", with spaced boluses of local anesthesia injected by syringe, produced only a slight decrease in bleeding. An article in the Journal of Plastic and Reconstructive Surgery in 1985 reported a complication rate of over 20%. In 1986, a group of complications in Houston made the national TV show "60 Minutes", greatly dampening the public enthusiasm for this procedure.

Eyelid Surgery
People who look at you usually notice your eyes before any other facial feature. Aging, heredity, and sunlight often become manifest in your eyelid skin. These changes may either consist of baggy, excess skin on the upper lids, or of 'tired bags' (fat pads) on the lower lids. These changes may make you look tired, sad, or older than you actually feel, especially earlier in the morning. However, eyelid surgery can be a very satisfying facial cosmetic procedure, since, in many cases, these problems can be corrected with pleasing aesthetic results. Your self confidence may indeed improve as your eyes look brighter, younger, and more awake.
Liposuction
Is it possible to remove unwanted fat with a method that is safer, virtually painless, affordable, and aesthetically effective? With the `tumescent technique', in which liposculpture is performed in the office under local anesthesia and light sedation, the answer is now a definite `yes'. This innovative method, developed by a California dermatologist and perfected by Dr. Swinehart since 1987, eliminates much of the risk associated in the past with liposuction - namely, bleeding, infection, high cost, and need for hospitalization. In addition, patients are now often able to return to work or normal activity in a matter of one to three days, in contrast to the lengthy recovery time required in years past.
Face Lift
Improperly performed face lift surgery may permanently damage hair follicles along the temples and sideburns: browlifts (forehead lifts) may move the hairline back to an unacceptably high location, and scars along the hairline may be quite visible when the hair is parted or when the wind blows. In these instances, a new hairline must be created with 1-hair and 2-hair micrografts or follicular units. Of course, a normal, dense donor area must be present on the sides and back of your scalp in order for the surgeon to perform successful hair replacement.

Body Contouring
You will be lightly sedated, though relaxed, but will not be under general anesthesia. The body contouring generally takes 2 to 3 hours to complete. The procedure is performed in an office surgical facility utilizing trained professional staff and containing state-of-the-art monitoring and safety equipment.
Laser Resurfacing
As with all forms of surgery, a scar will remain after the skin cancer is removed and the surgical area has completely healed. Mohs micrographic surgery, however, will leave one of the smallest possible surgical defects and resultant scars. Often, wounds allowed to heal on their own result in scars that are barely noticeable. Even following extensive surgery, results are frequently quite acceptable. In addition, scars do have the ability, through the body's own natural healing properties, to remodel and improve in appearance for a six to twelve month period. There are also many other techniques available to the patient for enhancement of the surgical area following skin cancer surgery. Depressed or indented scars may be elevated, using an implant such as Zyderm collagen. Likewise, a raised or roughed scar may be smoothed, using laser resurfacing or chemical peeling techniques. Skin flaps and grafts also may require a subsequent "touch up" procedure, to further improve their appearance.
Cosmetic Surgery
In response, Jeff Klein, a California dermatologic surgeon, invented the "tumescent technique" in 1985 and 1986, publishing his landmark paper in 1987 in the American Journal of Cosmetic Surgery.
Dermabrasion
CHEMICAL PEELING involves the application of trichloracetic acid, phenol, glycolic acid, and other agents to the skin to treat and remove fine wrinkles and sun damage. With DERMABRASION, the dermatologic surgeon sands the skin (which has been numbed) with a rotary mechanized wheel, removing the acne scars, wrinkles, or sun damage.

Ultraviolet light increases 4% for every 1,000 feet in altitude. Therefore, in Denver, we have approximately 20% more ultraviolet, and at 10,000 feet, we have 40% more. Add that to the outdoor lifestyle, reflection from the snow, and sand, and 300 days of sunshine per year, and our skin cancer rates are among the highest in the nation!

At Creighton University School of Medicine, he ranked third in his class of 85, graduating Cum Laude (with honors) as a member of Alpha Omega Alpha, the national medical honor society. During his residency at the University of Colorado, Dr. Swinehart received his specialty training, and was Chief Resident in Dermatology in 1977-1978.

Fungus Toenails
Do topical antifungals work for this condition? Some do, but to a very limited extent; the best topical is a mixture of thymol in chloroform, available only by prescription. Nails are like steel- very few chemicals can penetrate them. The best results for treatment of toenail fungus are seen with prescription oral antifungals, and these must be taken continuously for up to 12 months.
