Western Connecticut Home Care (WCHC), formerly Danbury Visiting Nurse Association, was founded in 1911. WCHC is the home care affiliate of Western Connecticut Health Network which includes Danbury Hospital, New Milford Hospital and Norwalk Hospital. We are licensed by the State of Connecticut, certified by The Centers for Medicare and Medicaid Services (CMS) and accredited by The Joint Commission National Quality Assurance

Western Connecticut Home Care provides nursing, rehabilitation therapy and medical social work to adult patients, of all ages, who need care after an illness, injury or surgery or for a chronic condition. Our comprehensive Maternal/Child Health program provides medical care and support to expectant mothers, newborns and their families. We also offer public health services such as flu vaccinations and health screenings, and Philips Lifeline, medical response system. Our home health care services include: Skilled Nursing * Rehabilitation services, including physical therapy, occupational therapy, and speech therapy * Medical Social Work * Home Health Aide Services Our programs include: Chronic Disease Care and Management, Rehabilitation Therapy, Infusion Therapy, Wound Care, Fall Prevention Assessment, Palliative Care, Maternal Child Health Care, and Telehealth.
