Back To Sleep campaign was initiated by the American Academy of Pediatrics in 1992, with hopes of lowering the incidence of SIDS (Sudden Infant Death Syndrome), physicians noted that more and children were showing up with flat areas on the backs of their heads. Early on, many children with flat heads were incorrectly diagnosed as having craniosynostosis, and a number underwent unnecessary surgery.
Ear Infections
When a child is born with an absent ear, keeping the opposite functional eardrum healthy takes on an even greater importance. I recommend that parents who have a child with microtia be on the lookout for any sign of an ear infection on the opposite side (it is not really possible to get an ear infection on the microtic side). Multiple ear infections on the unaffected side may diminish hearing; therefore, parents should have a low threshold for bringing their baby to the pediatrician (or pediatric Ear, Nose and Throat specialist) to check for possible ear infections. In general, the vast majority of children with microtia never need any hearing aids. It is important to have your child's hearing tested periodically to ensure the health of the unaffected ear.
Emergency Care
Tarsorrhaphies might be needed. This small operation entails sewing the upper and lower eyelid together near the outside corners of the eye. This is actually not permanent and is a completely reversible operation (if done in the proper way). For children with large bulging eyes, I recommend that parents keep a tube of Lacrilube or Genteal ointment at home to moisturize the eyes until a tarsorrhaphy can be performed. Should your child’s eyelid get trapped behind the eye, if you are unable to quickly pull the upper eyelid back over the globe (with a wet Q-tip under the eyelid) then put lots of ointment over the entire eye (to prevent drying and scarring) and go directly to the emergency room.

The first step in the treatment of a child with suspected PRS is to determine if the child actually does have PRS and if so, is it isolated PRS or is it associated with another syndrome. PRS features can occur as part of many different conditions and syndromes, and it is very important that a genetic evaluation and testing be performed. The most common syndrome associated with PRS is Stickler syndrome. Stickler syndrome is sometimes associated with joint problems, arthritis, and with multiple eye issues (including myopia, retinal degeneration and detachment, early cataract formation, and a higher incidence of glaucoma) and this condition needs to be accurately diagnosed so that affected individuals can be closely followed by an ophthalmologist. Less commonly associated with PRS is Velocardiofacial syndrome (also known as: VCFS, 22q11 deletion, or by the older term, Shprintzen syndrome) and Fetal Alcohol syndrome. A genetics consultation is an important part of the initial treatment of a child born with PRS.

It is important for urologists to bring the testicles down into the scrotum, in order to improve the ability to monitor for the low but significant risk for testicular cancer. MRI
One constant concern for children born with Pfeiffer syndrome is the possibility for raised intracranial pressure. We know that children with Pfeiffer syndrome have larger brains. Although the skull can get bigger without sutures, the abnormal closure of skull sutures (craniosynostosis) does somewhat limit the ability for the brain to enlarge freely in all directions, which can lead to slightly higher pressures inside the skull, which in turn may reduce blood flow to the brain. Raised intracranial pressure is most often treated by surgically enlarging the skull (see Treatment below), an operation that usually has to be done somewhere between 2 and 4 times during a child’s lifetime, depending upon when the first operation is done and how well it is performed. Currently, no one is sure exactly what the ideal age is for the first skull enlargement operation. Studies have shown that most craniofacial surgeons perform the first skull enlargement around 6-months of age. Our research suggests that doing surgery at this early age is not only unnecessary, it is too soon because it impairs the future growth of the skull, often to the extent that additional operations will need to be added in the future. In Dallas, we believe it is safe to delay the first skull surgery well beyond 6-months of age to somewhere around 15-months of age, depending upon the degree to which the skull is affected. Not only are we convinced that this delay is safe, we are convinced that by waiting until a child is older before enlarging the skull, we can reduce the total number of lifetime operations. Sometimes parents are told that their child has raised pressure and must have surgery right away. It is very important for parents to find out exactly what has convinced their doctor that urgent early operations are necessary. Our experience suggests that it usually takes quite a while for raised intracranial pressure to have any adverse effects, and most often the biggest effect is on vision, not on mental development. However, it is important that growing children be closely monitored by experienced physicians to make sure that the intracranial pressure is not getting too high. This monitoring is done a number of ways: having a pediatric neuro-ophthalmologist evaluate the optic nerves (raised pressure can cause bulging of the nerve in the back of the eye), following head circumference measurements with growth, monitoring changes in the MRI scans, performing “visual evoked potentials” (a measurement of the speed that light travels from the eye nerve to the brain) or “optical coherence tomography” (a measurement of the thickness of the back of the eye), monitoring overall intellectual development, and looking for signs in older children such as headaches or vomiting. Lastly, it is even possible to put a tiny catheter under the skull to directly measure pressure.

Although many centers routinely order plain x-rays or even CT scans, the diagnosis of positional plagiocephaly (or skull deformation) can be easily made by just doing a careful physical exam without ordering any studies. Children with deformations will usually have flattening on one side of the back of the skull, although both sides can be affected. Often the forehead, on the same side of the back flatness, is further forward and sometimes the cheekbone will also be more prominent on this side. When the back of the head is flat on one side, and on the same side the forehead is further forward, we refer to this as a parallelogram deformity. Sometimes this is more obvious when you lightly place your index fingers in each ear canal in order to see which side is further forward. It is a lot more common for the right side to be flat than the left, and this condition is also more common in twins. It is important to make sure that the flatness is coming from an outside pressure and not the result of craniosynostosis (specifically, a fused lambdoid suture). One of the key ways to tell which condition is causing the flatness is to look at the height of the skull on the flat side. With positional plagiocephaly, the skull height is normal but with craniosynostosis the skull height is reduced. Other signs of craniosynostosis are a low skull bulge behind the ear on the same side that is flat, and compensatory overgrowth of the opposite side, with the skull bulging off to the side (see the section on lambdoid synostosis ). As a rule of thumb, if the height of the back of the skull on the flat side is the same as the other side, the child almost certainly has positional plagiocephaly.

General Surgery
Dr. Fearon was born in the United States and grew up in London, England, until his family moved back to Connecticut. After graduating from the Mt. Hermon School in upstate Massachusetts, he received his B.A. from Brown University. He then pursued post-baccalaureate studies at Columbia University before attending medical school at the University of Cincinnati. Dr. Fearon went on to complete a full six-year general surgery residency at the Deaconess/Harvard Fifth surgical service and then a plastic surgery residency at the Massachusetts General Hospital in Boston, both associated with Harvard Medical School. After completing a fellowship in craniofacial surgery at the Children’s Hospital of Philadelphia with the University of Pennsylvania, he joined the Dallas Craniofacial Center as a director. Dr. Fearon’s practice is focused on the treatment of rare birth defects in children, who come to Dallas from across the United States, as well internationally. He remains actively engaged in craniofacial research, has authored many scientific articles and book chapters, and is a former Associate Editor for the Journal of Plastic and Reconstructive Surgery. Dr. Fearon is a president emeritus of both the Texas Society of Plastic Surgeons, and of the American Society of Craniofacial Surgeons, and has been awarded a U.S. patent for a bone distraction device. In addition to having served on a number of community boards, he is an amateur sculptor, enjoys helicopter skiing, and is an avid surfer.
Orthopedics
This term refers to any technique that is used before surgery to move both sides of the cleft in order to narrow or better position each side, with the goal of making the eventual repair easier. There are a number of different types of presurgical orthopedics that might be recommended. The most simple of these is a specialized elastic tape, which is stretched across the cleft on top of the cheek skin in order to slowly bring the two sides of the cleft lip closer together. Although this requires keeping tape across a baby’s lip, it can be surprising how just over the course of a few weeks the two sides will come so much closer together. For babies born with bilateral clefts, this taping can help better position the central part of the lip, called the
Ear Surgery
Following outer ear reconstruction, it is also possible for children to undergo inner ear reconstruction (by a specialized ENT) in order to rebuild the absent eardrum. This is done to try to improve hearing on the affected side, and achieve some stereo hearing that improves the localization of sound. This operation is not without some downsides that need to be discussed with your doctor. I also strongly recommend speaking with other patients who have undergone inner ear reconstruction before deciding to proceed with inner ear surgery. For those who decide to proceed with inner ear surgery, it is important to first finish building the external, or outer ear.
