Cervical spine myelopathy occurs when age-related osteoarthritic changes cause cervical spinal stenosis, a narrowing of the cervical spinal canal leading to cord compression and disability. Other age-related changes that cause cord compression include disc herniation or bulging discs; and ligament hardening causing bulging into the canal. Other causes include Rheumatoid arthritis, spinal fractures, and injuries to the neck.
Fibromyalgia
Musculoskeletal pain refers to pain originating from the muscles and the skeletons around the spine. Examples of this kind of pain include most athletic injuries and such syndromes as fibromyalgia and myofascial pain.
Medical Weight Loss
Initial treatment for spinal stenosis is typically conservative management with various medications including mild analgesics, non-steroidal anti-inflammatory agents and sometimes use of cortisone, either orally or by injection. Physical therapy is frequently effective along with weight loss for nonoperative management of spinal stenosis. However, if the pain is not controlled or progressive weakness develops, surgery is then considered as a treatment option.

Carpal Tunnel Syndrome
Carpal tunnel syndrome is caused by elevated pressure in the carpal tunnel which is located at the base of the palm, just beyond the wrist crease extending into the hand. The tunnel is formed on three sides by the wrist bones, which create an arch, and on the palm side by a large ligament called the transverse carpal ligament. The median nerve passes through the tunnel, and increased pressure produces a lack of blood supply to the nerve, resulting in impaired nerve function (electrical conduction) and attendant numbness, tingling and pain. Early in the course, no morphologic changes are observable in the median nerve, neurologic findings are reversible, and symptoms are intermittent. Prolonged or frequent episodes of elevated pressure in the carpal tunnel may result in more constant and severe symptoms, occasionally with weakness. When there is prolonged ischemia or lack of blood supply, axonal injury ensues, and permanent nerve damage may occur.

Cervical radiculopathy is the medical term for irritation/impingement of any nerve root originating in the neck. It is characterized by neck pain and pain that radiates from the neck into the shoulder and may have associated numbness, tingling and weakness in the arms and fingers. Significant functional limitations and disabilities are common. Cervical radiculopathy can be a disabling condition that has a significant negative impact on the quality of life, mental health, physical function, and social participation. Cervical radiculopathy is common in individuals with degenerative changes in the spine due to aging. 80% of cervical radiculopathy affects C6 or C7 nerve roots.

A variety of conditions may be associated with carpal tunnel syndrome. These include pregnancy, inflammatory arthritis, Colles’ fracture, amyloidosis, hypothyroidism, diabetes mellitus, acromegaly and use of corticosteroids and estrogens. Up to one third of cases of carpal tunnel syndrome occur in association with such medical conditions; about 6% of patients have diabetes. Carpal tunnel syndrome is also associated with forceful, repetitive activities of the hand and wrist. Occupations associated with a high incidence of carpal tunnel syndrome include food processing, manufacturing, logging, construction work and repetitive tasks such as typing.

A diagnosis of Parkinson’s disease is usually made clinically by the observation of symptoms by a trained neurologist. Recently, the FDA has approved a diagnostic test for Parkinson’s disease. The DaTscan is a nuclear medicine test, which can show a loss of dopamine neurons in the substantia nigra. It may be useful in distinguishing Parkinson’s disease form essential tremor.
Computed Tomography
At your neurosurgical consultation, your Dallas Neurosurgical & Spine expert will take a thorough history, ask about your symptoms, the quality of the pain, the treatments that have been received, and how the pain affects your activities. They conduct a physical examination including testing reflexes, muscle strength, and sensation. A neurological exam can help to identify the level of the herniation and it may be all that is necessary to diagnose a herniated disc. Imaging tests including x-rays, CT scans or MRIs will be ordered to identify other conditions that could cause or contribute to your pain, and to visualize the spinal cord and affected nerves.
Chemotherapy
Aggressive treatment utilizing surgery, radiation and chemotherapy can often control the growth of the tumor for a period of several months to several years. In general, the survival rates for this tumor are much higher in patients less than 50 years of age.
Radiation Therapy
Meningiomas do not always require immediate treatment. A small meningioma that causes no significant signs or symptoms may be monitored over time for signs of growth. A small meningioma that never grows may never require treatment. For tumors that are larger or are causing signs and symptoms, surgery is often recommended. Meningiomas that arise in the brain are often treated through a routine craniotomy for resection of the lesion. Spinal meningiomas may also be removed through routine laminectomy approaches. At times, surgery is unable to completely remove the meningiomas of the brain. This may be due to the meningioma occurring near delicate structures. In those cases, the meningioma is removed as completely as possible. Residual meningiomas may be treated with either radiation therapy or radiosurgery.

Trish graduated from South Dakota State University in 2003 with a Bachelor degree in Nursing and worked as a nurse in general surgery for years before returning to school for her Master of Science in Nursing degree which she obtained from Saint Louis University in 2012. After 10 years of surgical experience, she went on to complete an RN First Assistant program to enhance her surgical skill set in 2013. She holds certification with the American Academy of Nurse Practitioners. In addition to assisting in surgery, Trish collaborates with Dr. Krumerman rounding on patients in the hospital as well as seeing patients in the office.
Sciatica
Sciatica is caused by irritation or compression (pinching) of the sciatic nerve or the lumbar spinal nerve roots that merge to form the sciatic nerve. Sciatica typically manifests as pain radiating from the buttock down the back of the thigh into the calf and/or foot. The word “sciatica” derives from the Latin word “ischiadicus”, which means pain in the buttocks/hip region. Sciatica is often associated with numbness and tingling in addition to the pain symptoms. Occasionally, patients may also have some associated weakness. Sciatica is often a form of radiculopathy (please see Radicular Syndrome for more details).
Pinched Nerve
Radiculopathy is dysfunction of a nerve that originates in the spine. This dysfunction is usually caused by compression of the nerve, known as a “pinched nerve” in lay terms.

Botox
The typical workup for hemifacial spasm involves obtaining a special MRI through the brainstem, looking for a branch of a blood vessel, tumor, or vascular malformation. If a tumor or vascular malformation is identified, then appropriate treatment may involve surgery or endovascular treatment (the treatment of a vascular lesion using minimally invasive techniques through the groin). If a blood vessel is identified as the cause, the primary treatment is either Botulinum Toxin (Botox) or surgical decompression of the nerve. Treatment of hemifacial spasm with Botox involves the injection of the toxin in the face. Treatment is often successful but requires repeated office procedures every few months.

Treatment of back pain is often conservative and limited to anti-inflammatory medication and physical therapy. Persistent back pain, or back pain associated with weakness or radiating pain, may require further intervention. Surgical intervention is often beneficial, but it is necessary in certain conditions only. Dallas Neurosurgical & Spine employs a conservative approach to all back pain patients. Physical therapy, along with anti-inflammatory medications, muscle relaxants and medicines that reduce nerve irritation are mainstays of treatment plans. They are tailored specifically to a patient’s condition. Once the more conservative treatment modalities have been tried and/or the severity of the symptoms requires further intervention, some of the following pain treatments may be appropriate.
Slipped Disc
Also called a protruding disc, ruptured disc or slipped disc, a herniated disc occurs when the tough outer layer weakens and tears. This results in some of the inner, gel-like material to protrude into the spinal canal, compressing the spinal cord or irritating and impinging nearby nerve roots. Cervical disc herniation is a common cause of neck and arm pain in adults.
